1.Have a safe and supportive place to live
2.Participate in meaningful activities
3.Maintain access to needed services
4.Develop friendships and intimate relationships
This presentation includes a discussion, with case examples, of evidence-based strategies to support positive transition outcomes for persons with complex communication needs.
